Liberty Place can help you with our Assertive Community Action Team (ACAT) Program. Our ACAT stands apart from all others. Our service delivery means meeting you in the community, meeting you where you are. We meet our clients where they are presently – at any location and/or intersection.
We do not just work in the office!
Our ACAT case managers will assist you with paperwork, go with you to your appointments – to make sure you get the right services for your needs. Our team provides you with a link to various services that will help build an individual’s (or families’) capacity towards housing security and stability. Our case managers utilize a client-centered approach to help individuals achieve personal goals for their well-being.
